Quick Apply Full-time 1 day ago Full Job Description The Registered Nurse Assessment Coordinator (RNAC) is responsible for the completion & submission of MDS assessments and other required reports.
Complete residents MDS Assessment Reports, annual & quarterly reviews, Raps, and MA 103 as needed.
Overall management and coordination of the care plan process in accordance with HCFA guidelines.
The RNAC is responsible for the coordination of information for resident's care plans.
The RNAC ensures accurate, current information, for resident's care plans.
The RNAC ensures accurate, current information, which meets standards and regulations that is provided to third party payers to facilitate reimbursement for resident's care.
The RNAC provides the function of ensuring the daily treatment/care provided to the resident that is appropriate to the resident's diagnosis and condition and meets the specifications of physician's and therapist's recommendations.
Involved in IDT meetings for resident care and progress.
The IDT process is designed to allow team members to review and discuss information and make recommendations that are relevant to the resident's needs.
Serve as a member of the Quality Assurance Team, and on any other appropriate committees as requested and attend/participate in staff meetings.
Regular schedule Monday thru Friday with occasional weekends for manager on duty.
